2012 WBSI General Forms
Completed forms can be mailed to WBSI PO Box 4462 Warren NJ 07059 Applications processed after January 31st require a $50.00 late fee and after February 15th to the 29th a $100.00 last chance fee. Players after January 31st will be placed onto a waiting list and may not be accepted, depending on league and team size considerations. Refunds granted before evaluations will be charged a $25.00 fee, half registration fee after evaluations. No refunds will be granted after the season begins. WBSI does not require any additional fee’s, performance bonds, booster bonds, fund raising or candy sale quotas, snack shack or work crew labor in addition to your registration fee.
